Tesamorelin is a 44-amino-acid analogue of growth hormone-releasing hormone (GHRH) with an N-terminal modification designed to resist enzymatic cleavage. It is studied through the GHRH-receptor pathway, which distinguishes its research mechanism from ghrelin-receptor agonists such as Ipamorelin.

Both are GHRH analogs but engineered differently. Tesamorelin is full-length GHRH(1-44) with a single N-terminal modification, preserving native receptor binding kinetics. CJC-1295 is truncated GHRH(1-29) with four substitutions to extend half-life. Different research designs favor different profiles.
